Enlisted Shepherds Bush, London, W, age 18 years & 150 days, height 5ft 8 inches, weight 126lbs, fair complexion, blue eyes, brown hair, scar under left arm, scar under left ribs. Formerly pipe jointer (Metropolitan Water Board).

Separation Allowance of 4/3 a week & allotment of Pay of 3/6 a week payable to father.
