11th December 1915, enlisted & posted Depot, Royal West Kent Regiment; 11th December 1915, posted 10th Battalion, Royal West Kent Regiment; 3rd May 1916, posted BEF; 3rd May 1916, Southampton, embarked; 4th May 1916, Havre, disembarked; 13th June 1916, 140 Field Ambulance, impetigo; 23rd June 1916, to Duty; 2nd July 1916, Boulogne, transferred to England, ex No.13 Stationary Hospital, per Hospital Ship, Cambria, shell/shrapnel wounds, neck, face & back; 1st July 1916, wounded;
2nd July 1916 to 13th July 1916, Military Hospital, Endell Street, WC, when wounded was unconscious for a short time, not sick, has suffered from severe headaches since, wound of right occipital region, probe passed to ???? bone and fracture felt septic, septic wound of nose and fractured nasal bones, multiple wounds of right side & left shoulder septic, two ??? wounds right arm, paresis of both legs & severe pain down right side & right leg, draws up right leg and cries out in pain, 3rd July operation, depressed fracture of scalp, wound septic, penetrated brain, hemorrhage, 5th July severe headaches, 6th July, lumber puncture, fluid drained, pus present, 8th July, delirious, 11th July, still semi conscious, 2nd lumber puncture, fluid less, 12th July, pulse still fair, 13th July, unconscious, pulse feeble, patient died at 10.30am.
