Enlisted Leeds, Yorkshire, age 27 years & 6 months, height 5ft 6¼ inches, weight 126lbs. Resided 13 Burton End, Haverhill, Suffolk. Formerly tailors cutter.

12th February 1916 to 3rd January 1917; BEF 4th January 1917 to 14th June 1917.

Separation Allowance of 7/1 a week & Allotment of Pay of 3/6 a week payable to Matilda Mead, grandmother.

12th February 1916, enlisted; 13th February 1916, to Army Reserve; 10th March 1916, mobilized Tonbridge, Kent, posted 3/1st Kent Cyclist Battalion, Service No.2444; 27th May 1916, posted 2/1st Kent Cyclist Battalion; 3rd January 1917, embarked / disembarked; 4th January 1917, posted 11th Battalion, Royal West Kent Regiment; 9th February 1917, joined 11th Battalion, Royal West Kent Regiment, In the Field; 15th May 1917, In the Field, insolence to his Company's Officer, severely reprimanded.

26th September 1919, Statement of Relatives, completer by Matilda Head, grandmother - Father, Charles Mead, has been in Melton Asylum, near Woodbridge, Suffolk,  for the last 29 years; Mother, none; Brothers, none; Sisters, none; Grandparents, Matilda Mead, 13 Burton End, Haverhill, Suffolk; Nephews/Nieces, nil; Uncles / Aunts, Hephizilah Hazelwood, 50 Primrose Hill, Haverhill, Suffolk.

Commemorated at Ypres (Menin Gate) Memorial, Panel 45 and 47, Belgium & on Haverhill Roll of Honour & Haverhill War Memorial.
