Enlisted Penenden Heath, Maidstone, Kent, age 38 years & 10 months, height 5ft 4½ inches, weight 123 lbs. Resided Rose Cottage, Icklesham, Sussex. Formerly gentleman's servant, valet.
Home 7th October 1915 to 2nd May 1916; BEF 3rd May 1916 to 31st July 1917.
Separation allowance of 5/3 a week and allotment of pay of 3/6 a week paid to mother, Mrs H Butler, Rose Cottage, Icklesham.
