Enlisted 22nd October 1915, Lewisham, age 37, height 5ft 6 inches, resided 72 Crescent Road, Clapham, SW4. Formerly saddler.

Husband of Annie Grace Collins, (nee Wright), 72 Crescent Road, Clapham, SW4 / 14 Bromwell Road, Clapham, SW4. Married 6th April 1902, Clapham.

22nd October 1915, specially enlisted at 5/- per diem (Latin for per day) into Royal Engineers, 134466, as skilled harness maker; 19th November 1915, 5th "P" Company, deserted; 16th February 1916, apprehended by Civil Police; 24th February 1916, handed over to Military Authorities; 25th  to 29th March, awaiting trial; 30th March 1916, tried by Court Martial for desertion, sentence to 140 days detention; 16th June 1916, sentence expires; 17th June 1916, returned to Duty; 7th July 1916, deserted; 17th August 1916, returned from Desertion; 17th August to 28th August, in close arrest; 29th August to 4th September 1916, in Guard Detention Room awaiting disposal; 4th September 1916, tried by Court Martial for desertion, sentenced to 9 months hard labour, sentenced varied to read 6 months detention; 1st March 1917, transferred to 3rd Battalion, Royal West Kent Regiment; 4th March 1917, sentence expires.

24th March 1917, Medical Board Report - disability pulmonary tuberculosis - originated February 1917, Chatham, causation, exposure to wet and cold, breathless on slight exertion, always a little temp a slight rise at night, sputum contains TB, loss of weight, nights sweats, severe cough, not the result of or aggravated by Service. Prevents 50% for 3 months.

14th April 1917, discharged  as "no longer physically fit for War Service" due to wounds. Silver War Badge issued

Gratuity £5.00

6th October 1917, Parliamentary War Pension Committee appeals on behalf of man. Mans present condition may be regarded as aggravated by |Service since the declaration of War. Pension of 13/9 a week from 14th April 1917 in lieu of former award. Expires 15th October 1918.
