Born 1892, Clay Cross, Derbyshire, enlisted 10th December 1915, age 23, height 5ft 4 inches, weight 140lbs, artificial teeth, resided Market Street, Clay Cross, Chesterfield. Formerly builders foreman.

Previously served with Sherwood Foresters, 83131.

2nd March 1918 to 23rd March 1918, 3rd Northern General Hospital, Sheffield, indigestion, on leave from BEF, vomiting for last three weeks, some dental treatment, returned to duty; 12th April 1918, wounded, gun shot wound to face, in front of right ear.

30th September 1918, Medical Report, slight stiffness of jaw, operation to remove bullet fragment, parietal gland, right side; 22nd July 1919, Medical Report, swelling in region of wound, tenderness present, can only open mouth 2/3 normal size. All his teeth have been extracted, but not on account of wound. Slight facial disagreement. Headaches & sleeplessness; 24th August 1920, Medical Report, mild pains in head, sleeplessness, inability to open mouth properly, scars healed; 18th September 1920, Medical Report, face due to 20% for 12 months; 10th August 1921, Medical Report, face due to 20% for 12 months;

20th January 1919, discharged as "no longer physically fit for War Service" due to wounds. Silver War Badge issued
