18th August 1917, Medical Report, Cedar Lawn Military Hospital, Hampstead Heath, NW3 - Had attack of bronchitis at Aldershot 1915. Became dyspnoeic and had cough in June 1917 After 3 weeks in St Omer Hospital he was transferred to Metropolitan Hospital where he had albumen appear in urine. He was transferred to Cedar Lawn 12th June 1917. He has had albumen in his urine during the two months he has been here and he has had bronchitis - emphysema marked. General condition has improved somewhat. Nephritis attributable active service (exposure). Present condition generally poor. He has marked sign of bronchitis and emphysema. There is a .1% of albumen in his urine on fish chicken etc. On the least exertion such as walking upstairs he becomes dyspnoeic. He is unfitted for the duties of a soldier.
13th September 1917, discharged as "no longer physically fit for War Service" due to sickness. Silver War Badge issued
Pension 13/9 a week for 13 weeks then 6/3 to be reviewed in 39 weeks. Disability, nephritis, bronchitis, emphysema.
Pensioner deceased.
