Born Plaxtol, Kent, son of Mrs. Helena M Levett, 11 Spencer Cottages, Station Road, Borough Green, Kent / The Spout, Plaxtol, Kent.
Enlisted Tonbridge, Kent, age 18 years & 6 months, height 5ft 8¾ inches, weight 151lbs, fresh complexion, light brown eyes, dark brown hair, scar on knuckle 1st finger right hand. Formerly farm labourer.

4th January 1912, enlisted & posted Depot, Royal West Kent Regiment; 17th April 1912, posted 1st Battalion, Royal West Kent Regiment; 6th September 1912, having a dirty rifle on Parade, 10 days confined to Barracks; 5th November 1912, not leaving his bed cot tidy when ordered to do so, 5 days confined to Barracks; 21st December 1912, passed in Transport Duties; 3rd February 1913, Dublin, having a greasy great coat collar & dirty equipment on Guard mounting Parade, 3 days confined to Barracks, 3 extra guards; 8th February 1913, Dublin, having dirty boots on Guard mounting Parade, 3 extra guards; 12th February 1913, Dublin, having dirty trousers on Guard mounting Parade, 5 days confined to Barracks; 19th February 1913, slackness when a sentry on vice Regal Guard, 10 days confined to Barracks; 27th February 1913, 3rd Class Certificate of Education; 25th June 1913, slackness on Vice Regal Guard, 10 days confined to Barracks & 3 extra Guards; 24th July 1913, Dublin, not complying with an order, viz. not having laces at grenades inspection, 7 days confined to Barracks; 22nd August 1913, Dublin, losing bayonet & dirty equipment when parading for route march; 16th October 1913, Dublin, overstaying his furlough from 10.00am until arrested by the Civil Power until 3.00pm on same date, 10 days confined to Barracks; 22nd October 1913, posted 2nd Battalion, Royal West Kent Regiment; 14th February 1914, Multan, irregular conduct on Parade, 5 days confined to Barracks; 4th December 1914, Hydrerbad, stating a falsehood to Colour Sergeant Walker, 7 days confined to Barracks; 27th December 1914, Hydrerbad, absent from Tattoo Roll Call 26th December 1914 until 10.25pm 26th December 1914, 5 days confined to Barracks; 31st January 1915, Karachi, embarked; 6th February 1916, Basrah, disembarked; 6th February 1915, posted Asiatic Theatre of War; 25th August 1915 to 12th September 1915, Nasiriyah Hospital, dysentery; 25th September 1915 to 2nd October 1915, Nasiriyah Hospital, tonsillitis; 6th December 1916, unofficially reported died whist a Prisoner of War. Captured at Kut al Amarah, 29th April 1916, informant Pte F G Booth, Service No.9131, Royal West Kent Regiment, Prisoner of War at Adana, Turkey.
