Born Mereworth Kent, son of Mathew Thomas and Mary Ann Ford, of Mereworth, Maidstone, Kent.

1913 June Quarter, Malling Registration District, married Annie May Crittenden, spinster, of 8, Cascade Avenue, West Malling, Kent.

Enlisted Maidstone Kent, age 26 years & 60 days, height 5ft 8¾ inches, weight 123lbs, various cavities, to see dentist.  Resided 1 Mayell Cottages, Bullfields, Snodland Kent. Formerly Assurance Agent.

11th December 1915, enlisted; 12th December 1915, to Army Reserve; 11th January 1916, mobilized & posted 12th Battalion, Royal West Kent Regiment; 2nd July 1916, Aldershot, overstaying his pass from midnight until reporting himself at 7.55am 3rd July, admonished, forfeits 1 days pay; 1st September 1916, posted 3rd Battalion, Royal West Kent Regiment; 21st November 1916, Fort Pitt, Chatham, upper denture provided; 5th December 1916 to 8th December 1916, Drill Hall Hospital, Chatham, influenza, slight case; 30th December 1916, posted BEF; 30th December 1916, embarked / disembarked; 19th January 1917, joined 7th Battalion, Royal West Kent Regiment; 14th February 1917, 54 Field Ambulance, wounded in action, gun shot wound left arm, compound fracture, penetrating chest wound; 12th March 1917, 49 Casualty Clearing Station, died of wounds.

9th September 1917, date when Separation Allowance of 16/- a week and Allotment of Pay of 3/6 a week, payable to wife, will cease.

24th September 1917, pension of 18/9 a week awarded to widow & one child.
