7th June 1917, Messines, wounded; 20th June 1917 to 14th August 1917, Penrith Auxiliary Hospital, gun shot wound jaw; 15th August 1917 to 1st November 1917, Highfield Military Hospital Knotty Ash, Liverpool, gun shot wound jaw; 1st November 1917 to 30th January 1918, 1st Eastern General Hospital Cambridge, gun shot wound to head, fractured lower jaw severe;

9th January 1918, 1st Eastern General Hospital Cambridge, Medical report - gun shot wounds in action to head, shrapnel lower jaw fracture, severe, - pain & stiffness in deformed lower jaw, scars at neck, numbness of teeth, pain in swallowing, scars right hip, right shoulder, right forearm. Disability 50%. Man states was struck by shrapnel in head hip shoulder and arm, lower jaw broken. Has been to special jaw hospital.

30th January 1918, discharged as "no longer physically fit for War Service" due to wounds. Silver War Badge issued
