Enlisted Chelmsford, Essex, age 18 years & 10 months, height 5ft 11 inches, weight 151lbs, corns on two toes of right foot. Resided Cooksmill Green, Writtle, Chelmsford, Essex. Formerly fitter?

Separation Allowance of 2/10 a week and Allotment of Pay of 3/6 a week payable to mother.
