Born Eddleseboro, Buckinghamshire, enlisted 11th January 1915, Wood Green, Middlesex, age 37, height 5ft 4 inches, weight 122lbs, resided 1 Bath Cottages, Southgate Road, Potters Bar, Middlesex. Formerly labourer.

Husband of Alice Bates, 1 Bath Cottages, Southgate Road, Potters Bar, Middlesex.

Children - Alfred Henry Bates, born 9th May 1903; Horace William Bates, born 27th December 1905; Doris Evelyn Bates, born 31st January 1910; Phyllis Violet Bates, 2nd April 1912.

11th January 1915, enlisted; 13th January 1915, posted Depot, Royal West Kent Regiment; 16th January 1915, posted 8th Battalion, Royal West Kent Regiment; 4th April 1915, Redhill, absent from Tattoo until Tattoo 5th inst, deprived 10 days pay, forfeits 2 days pay; 30th August 1915 to 14th April 1917, BEF France; 14th April 1917 to 2nd July 1917, 3rd Western General Hospital Cardiff,  rheumatism.

Date & Place of Origin - 4th April 1917. Pains in limbs. Pains in limbs accompanied by attack of shivering. During the time he was in hospital in France complained of pains. On April 9th 1917 he was examined at the  3rd Western General Hospital Cardiff, when he complained of pains in the knees, legs and small joints of feet. Slight rise of temperature when in hospital. Caused by active service, exposure to cold and wet conditions, not permanent, probable duration 6 months

16th August 1917, discharged  as "no longer physically fit for War Service" due to sickness.  Silver War Badge issued

Pension of 8/3 a week to be reviewed in 26 weeks. Disability rheumatism, less than 20%. Allowance for children 4/6 for 4 weeks.
